Course Description

To establish, refine, and enhance a system-wide centralized post-acute health management system as a means of providing cost-effective health service delivery utilizing a full range of venues of care.

Course Objectives

  • To identify parameters of roles and responsibilities of the health manager to minimize political disruption while maximizing desired outcome.
  • To assure commonality of language, methodology and outcome expectations throughout the continuum by aligning the acute oriented care tracks with a post acute continuum.
  • To adapt an organizational philosophy of transitioning patient care from the most restrictive acute setting to least restrictive and less costly alternative venues including:
    • Subacute Care
    • Long Term Care Hospital
    • Skilled Nursing Facility
    • Home Health
    • Senior Care Clinics
    • General Outpatient Services
    • Other alternatives venues within the service capabilities of the system.
  • To allow for individual professional development for health managers.
  • To apprise health managers of the latest regulatory, clinical, and financial information as available in order to assist in informed decision-making regarding full utilization of the Health System Continuum of Care. Focus shall be on the most recent and most impacting regulatory changes including HIPAA and Patient Rights legislation.
